With the start of school just a few weeks away, the Knox County Public Schools are reminding parents and guardians of important health requirements that must be met before their child will be enrolled for the 2013-2014 academic year.
Students entering preschool or Headstart, kindergarten, sixth grade, or transferring into the Knox County school district for the upcoming year must comply with the health requirements of set forth by Kentucky State Regulations. Listed below are the health requirements based on age and/or grade. Documentation of each of these examinations or immunizations is required.
Physical Examination
- Must be completed upon initial entry to school (typically ages 3, 4, or 5).
- Must be performed up to 1 year prior to 6th grade entry.
Vision Examination
- Must be completed upon initial entry to school (typically ages 3, 4, or 5).
Dental Examination / Screening
- Must be completed upon entry to kindergarten.
Hearing and Vision Screening
- Must be completed upon entry to school.
Immunizations
- Must be completed upon initial entry into school. Documentation must provide proof that student is immunized against diphtheria, tetanus, pertussis, rubella, and poliomyelitis.
- Students entering the 6th grade must have a second varicella, TdaP, and meningitis shot.
All students enrolling in the Knox County Public Schools for the first time must provide a copy of their birth certificate, social security card, and the above medical documentation.
Parents/guardians that have not scheduled their child’s immunizations or examinations should call their family physician or Knox County Health Department soon to ensure that their child can be seen prior to the first day of school. State regulations and local policy mandate strict enforcement of these requirements.
